@article{oai:ssg.repo.nii.ac.jp:00000047, author = {坂上, 尚穗 and 大友, 篤 and 中江, 秀幸 and Sakagami, Hisao and Otomo, Atsushi and Nakae, Hideyuki}, issue = {1}, journal = {研究紀要 青葉 Seiyo, College review of Sendai Seiyo Gakuin}, month = {Sep}, note = {[目的]腹部術後症例における起き上がり動作時の術創部疼痛の軽減を目的に,日常生活自立高齢者の起き上がり動作における呼吸状態について調査した。[方法]対象者は日常生活自立高齢者20名(男性3名,女性17名,平均80.3±7.1歳)であった。課題動作は背臥位から端座位までと,側臥位から端座位までの2つの起き上がり動作とし,呼吸フローセンサーで換気量を計測した。また安静呼吸時の1秒間および2秒間の安静時換気量と頭部拳上後から1秒間および2秒間の動作時換気量を算出した。[結果]起き上がり動作開始時の呼吸状態は,背臥位から,および側臥位からの起き上がり動作とも吸気位の割合が呼気位よりも有意に多かった。安静時換気量と動作時換気量の比較では,背臥位からの起き上がり,および側臥位からの起き上がりとも1秒間および2秒間の安静換気量よりも動作時換気量が有意に減少した。[考察]吸気では横隔膜が収縮降下し,腹筋群が伸長され,静止張力が多く発生し,活動張力を減少にすることから日常生活自立高齢者における起き上がり動作開始時の呼吸状態を吸気位にし,腹筋群を伸長状態に保つために換気量を抑えていたと考えられる。「結論」日常生活自立高齢者において,背臥位からの起き上がり動作および,側臥位からの起き上がり動作とも,動作開始時の呼吸状態は吸気位で,動作開始後も換気量が減少していたことが示唆された。, Purpose: In order to reduce the surgical wound pain during sittng up in abdominal postoperative cases,we investigated the respiratory state of the daily life independent elderly during sitting up. Methods: The subjects were 20 daily life independent elderly (3 males, 17 females, average 80.3±7.1 years). The task movements were two sitting-up motions from the supine to the sitting and the sidelying to the sitting , and the ventilation flow sensor measured the ventilation volume. The resting ventilation volume for 1 second and 2 seconds during tidal breathing and the working ventilation volume for 1 second and 2 seconds after the head fist were calculated. Results: Regarding the breathing state at the beginning of the standing-up motion, the ratio of the inspiratory position was significantly higher than that of the expiratory position both in the supine and in the sidelying Comparing the resting volume and the working ventilation volume, both from the supine to the sitting and the sidelying to the sitting showed a significant decrease in the working ventilation volume compared to the resting volume for 1 and 2 seconds. Discussion: During inspiration, the diaphragm contracts, the abdominal muscle group is elongated, a large amount of resting tension is generated, and the active tension is reduced. It is thought that the ventilation volume was suppressed in order to maintain the extension state.
